Special Enrollment for Careplus and New Insurance Rates Announced by Frontier

2015 Special Enrollment Window: December 15 – 19, 2014 CarePlus, Supplemental and Dependent Life Insurance Plans Frontier has announced that they will be opening a  "Special Enrollment Window"  where you will be allowed to re-enroll  for the following coverages if you are not satisfied with the elections you made previously.   CarePlus Plan- Based on the agreement between CWA 1298 and Frontier, all CWA represented employees who transferred from AT&T to Frontier on October 25, 2014 are automatically enrolled in the CarePlus plan for a period of 36 months: ·        During this Window you are able to change your current CarePlus election for the 2015 plan year.    Important: If you waive CarePlus coverage during this Enrollment Window, you will NOT be able to enroll for the plan at a later date. Supplemental and Dependent Life Insurance – Since you participated in the 2015 AT&T AE Window, you viewed the AT&T rates for these plans.   The 2015 rates for the Frontier Life Insurance plans are different from the AT&T rates, and as a result you are able to re-enroll for these coverages during this Special Window. ·        Important: If your new election requires the Evidence of Insurability (EOI) approval, be sure to either complete the EOI application online during the enrollment Window, or when you receive a copy by mail from MN Life insurance company.  Your new election(s) will not go into effect until MN Life Insurance Company approve your application. We encourage you to view your CarePlus, Supplemental and Dependent Life insurance elections during this window.  You are not required to take Any Actions if you are satisfied with your current elections. Enrolling is easy: Online:  www.frontierbenefitscenter.com.  Follow the online instructions to enroll.  Always print or save a copy of your Confirmation Statement for your records since Confirmation Statements will not be mailed to your home.  By phone: You may call the Frontier Benefits Service Center at 1-855-FTR-2887, M – F, 9 a.m. to 6 p.m. Eastern, to enroll. Please save or print a copy of your confirmation statement, once you complete your elections.  If you do not have access to a printer, you may call the Frontier Benefits Service Center at 1-855-FTR-2887 to request a copy to be mailed or e-mailed to you.   Note: During this Window you are not able to make changes to your other coverages, the elections you made during the AT&T 2015 Enrollment window will stand.      Don’t miss the Window if you are planning to change your CarePlus, Supplemental & Dependent Life elections.  Once the Window closes, no exceptions will be made.      At this time please review all of your coverages to make sure that they are consistent with what you elected with AT&T.
