The Affordable Care Act (ACA) requires employers  to provide a Form 1095-C to all employees, former employees and dependents with health coverage*  You will need the information to complete your 2015 tax return. This is proof of health coverage which the ACA requires you to have or you will have to pay a tax penalty.   The 1095-C includes information on medical  coverage received through through your employer. The ACA requires all Americans to have qualifying health coverage or pay a tax penalty. Tax filers will include information from the Form 1095-C on their 2015 income tax return to establish whether or not they have coverage that meets this requirement.  If you have questions regarding this form please speak to your  tax advisor or access www.irs.gov.
